Feedback form - Draft Tweed Access and Inclusion Plan 2023 to 2026

Instructions for online screen reader users:

This feedback form has 5 parts and there are 10 questions in total. There is one question for each of the first 4 parts, which are free text entry answer. Free text entry is positioned below the question. Each question and answer is set out one after or below the other, not beside each other.

The fifth and last part asks questions about you to help us understand your answers better. You do not have to answer all the questions. There are 5 multiple choice questions with a radio button to select your answer. Radio buttons are positioned immediately to the left of the text.
